No Doctor-Patient Relationship Via Website

Important: Using this website, browsing its content, or submitting an appointment request does not create a doctor-patient relationship. A doctor-patient relationship is established only when you are physically examined and treated by a licensed healthcare provider at one of our clinic locations. Patient Responsibilities

As a patient, you agree to:

• Provide accurate and complete personal, medical, and insurance information
• Inform our providers of all medications, allergies, and existing medical conditions
• Follow treatment plans and recommendations provided by our healthcare professionals
• Arrive on time for scheduled appointments or provide at least 24 hours notice for cancellations
• Treat our staff and other patients with respect
• Notify us of any changes to your insurance, contact information, or legal representation
• Report any accidents or injuries to the appropriate authorities (police, employer) as required by law

Appointments and Cancellations

We offer same-day appointments for urgent injury cases. For scheduled appointments, we request at least 24 hours notice for cancellations or rescheduling. Repeated missed appointments without notice may result in a rescheduling fee or termination of the patient-provider relationship.

Online appointment requests submitted through our website are not confirmed appointments. Our staff will contact you within 24 hours to confirm your appointment date and time.

Payment, Insurance, and Billing

Accepted Payment Methods

We accept the following forms of payment:

• Most major health insurance plans (BCBS, Humana, Anthem, UnitedHealthcare, Aetna, and others)
• Auto accident/PIP (Personal Injury Protection) insurance claims

Insurance Verification

While we will verify your insurance coverage as a courtesy, it is ultimately your responsibility to understand your insurance benefits, deductibles, co-pays, and coverage limitations. You are financially responsible for any amounts not covered by your insurance, including but not limited to deductibles, co-payments, and non-covered services.

Auto Accident and Workers' Compensation Claims

For auto accident cases, we work directly with your auto insurance carrier and/or personal injury attorney. For workers' compensation cases, we coordinate with your employer's insurance carrier. In both cases, you may be asked to sign an assignment of benefits or an attorney lien. If your claim is denied or your case is settled without payment to us, you remain responsible for the balance of your account.

Medical Records and Documentation

Your medical records are maintained in accordance with HIPAA regulations, Kentucky state law, and our Privacy Policy. You have the right to:

• Access and obtain copies of your medical records (a reasonable fee may apply per Kentucky law)
• Request amendments to your records
• Request that records be sent to another healthcare provider or your attorney

To the maximum extent permitted by Kentucky law, our total liability for any claim related to our services shall not exceed the amount paid by you for the specific services giving rise to the claim.
